2025 Laity Service Award Nomination Form

The Laity Service Award provides the local church the opportunity to recognize a lay member (or married couple) whose commitment to service has been a key contribution to the life and ministry of the church and provides an encouraging example to other. Please use this form to submit a nomination before September 21, 2025. The Laity Service Award will be announced during the 11am worship service on Laity Sunday, October 19, 2025.
